The following is from the minutes of the September 22, 2010 meeting of the AILA DC chapter, at which Charlie Oppenheim spoke:
A question was raised from the floor regarding cases apparently approved before the priority dates are current. Mr. Oppenheim indicated that (as noted above) Service Centers request visa numbers when they process adjustments of status in advance of an applicant’s priority date becoming current. When the priority date is current, a visa number is authorized by his office, and the Service Center issues the welcome notice and orders the production of the Resident Alien card. In this situation, the approval is considered valid because a visa number has been assigned to the applicant.

However, there have been instances when a welcome notice has been issued, but a visa number was not requested/authorized for the applicant. In such cases, the Resident Alien card will not be issued until a visa number has been requested and authorized for the applicant. Mr. Oppenheim recommended that practitioners contact USCIS if they receive a welcome notice before the applicant’s priority date is current. USCIS should determine whether a visa number has been requested and authorized for the applicant, and if it is current and has not been requested, one should be requested immediately. If the applicant’s priority date is no longer current, the Service Center may assign to the applicant an otherwise unused, authorized visa number (visa numbers that have been assigned to a case that is later withdrawn, cancelled, or denied).

Last paragraph is very interesting.
